Monday & Tuesday: 8 a.m. – 5 p.m.
Wednesday: 8 a.m. – 6 p.m.
Thursday & Friday: 8 a.m. – 5 p.m.
Your scheduled appointment time has been reserved specifically for you. We request a 24-hour notice if you need to cancel your appointment. We are aware that unforeseen events sometimes require missing an appointment, and appreciate your cooperation. You will received a card in office for your appointment as well as email, text, or phone call reminders.
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. We are not responsible for how your company handles its claims or for what benefits they pay on a claim.
Your estimated patient portion must be paid at the time of service.
As a courtesy to our patients, we will bill insurance companies for services or pre-determine benefits for future treatment with your insruance company. If you have any questions, our courteous staff is always available to answer them.
We will file most dental insurance companies, but we are only participating providers with Anthem Dental, Delta Dental Premier, and Metlife Dental.
